I have seen sabotage several times (saddle strap cut, that sort of thing). Traps is a pretty common cause in RPGs but that might fall under blades, poison, etc. GRRM should be higher :)
A common cause of death on the Discworld is The Luggage. Killed a few dozen people over the books, if I'm not mistaken, and also eliminated many rare (now extinct) creatures.
That's definitely true for the first couple DW novels, though I would also classify it as suicide for those who thought they were going to steal Twoflower's gold.
Being a parent, especially the parent of someone with unique powers (Harry D, Harry P, Kvothe, etc.) or the mother of a princess or future princess that loves to sing.
I might change terrorism to old age:)
